- Appointment times: when can you come in for your three privates?, general times and days that work best for your schedule
- All body issues: List any injuries, past surgeries, neurological/systemic diagnosis (MS, rheumatoid arthritis, lymphedema, etc.), structural/anatomical issues (low back pain, disc pain, scoliosis, rotator cuff strains, osteoarthritis, hip replacement, knee replacement, etc.)
- Current state of movement/physical activity – Do you sit at a desk all day? Weekend warrior? Getting ready for a marathon? Golf? Ski? Tennis? Salsa Dancing?
